Muscle twitching is an important symptom to consider after a thyroidectomy, as it may indicate a parathyroid gland injury leading to hypoparathyroidism. The parathyroid glands are responsible for regulating calcium levels in the body. If these glands are damaged or removed during surgery, it can result in decreased parathyroid hormone (PTH) production, leading to low calcium levels (hypocalcemia). Hypocalcemia often manifests as neuromuscular irritability, which can include symptoms such as muscle twitching or spasms. This twitching is a direct consequence of excitability of the muscles due to insufficient calcium to facilitate normal nerve and muscle function.

Anorexia, hoarseness, and blurred vision, while they may be associated with thyroid surgery or other complications, are not specifically indicative of parathyroid gland injury and its effects on calcium metabolism.
